Mr. John Thomas Blake, 46, of Lumberton, passed away Monday, December 31, 2012 at Betsy Johnson Regional Hospital in Dunn, NC. He was born in Provo, Utah on July 31, 1966 to James David Blake and Marlene Thomas Blake.

Surviving are his parents, of the home; one brother, David Blake, of Lumberton; and also Jim and Lavella Dale, known to John as ??Ma? and ??Daddy,? who helped care for him while he resided in Dunn.

A funeral service will be held on Saturday, January 5, 2013 at 12:30 PM at The Church of Jesus Christ of Latter-day Saints, 450 Liberty Hill Road, Lumberton, with President John Ricks officiating. Burial will follow at Meadowbrook Cemetery.

Visitation will take place prior to the service on Saturday from 11:00-12:00, also at The Church of Jesus Christ of Latter-day Saints.
